It is always acceptable to call your provider if your child is ill. Please call at 9:00am on the day that you would like to be seen for an appointment. WE ASK THAT NO PATIENT TO COME IN TO THE OFFICE WITHOUT CALLING FIRST. WE ARE NOT A WALK IN FACILITY AND WE NEED TO BE PREPARED FOR EACH AND EVERY PATIENT ENTERING THE BUILDING.
IF YOU NEED FORMS COMPLETED, PLEASE FAX THEM TO OUR OFFICE AT 203-889-2249 OR CALL at 203-889-2297, PLEASE ALLOW US 14 DAYS TO COMPLETE ALL FORM REQUESTS. We provide each patient with a completed health form at the end of each physical - these become your responsibility to turn into daycares or schools.
